At Action Home Therapy, our intake care coordinator will help you identify your therapy options. Identifying the best therapy option for your lifestyle is important to maximizing each client’s potential. Outpatient home physical therapy fills the transition of care gap when Medicare patients are not homebound (a Medicare Part A requirement) but would benefit from therapy at home.
Criteria and Benefits for In Home Therapy
- Patients do NOT have to be homebound, unlike Medicare Home Health Care
- Medicare recipients who live in a senior living retirement community, private home/condo, residential care home or assisted living facility
- Limit in person contact to help avoid the spread of COVID-19
- Your time is important! With physical therapy at home, clients have no travel time.
- One on one therapy care where therapist gain insight of your residential space and can make suggestions to helping clients maintain a safe environment.
- Review of in home safety and suggest adaptive home modifications
